Appointment Request

Please use this form to request an appointment with us. While we will do our best to accommodate your requested day and time, please note your appointment is not booked until you get a confirmation from us. We see appointments on 20-minute intervals (9:00, 9:20, 9:40, etc.). The veterinarian is generally in surgery from 11 AM - 2 PM on Mondays, Wednesdays, and Fridays and seeing drop-off cases Tuesdays and Thursdays from 12 PM - 2 PM so please keep that in mind when requesting appointment times. A staff member will be back in touch with you as soon as possible.

For virtual consultations, please call the office after your appointment is confirmed and ask to have it converted to a Rapport video appointment. Note that we will no longer be using TeleVet.
